What this site is
This site is a source-grounded public-record index of the U.S. Department of Health and Human Services, Office of Inspector General (HHS-OIG) List of Excluded Individuals/Entities (LEIE). The LEIE identifies individuals and entities that are excluded from participation in Medicare, Medicaid, and other federally funded health care programs.
What this site is not
This is not an official government record and is not affiliated with HHS-OIG. Name matches alone are not sufficient to verify identity. You must verify identity and current exclusion status directly with HHS-OIG before relying on this information for employment, contracting, credentialing, legal, or compliance decisions. Absence of a record here does not prove that a person or entity has no exclusion, disciplinary, licensing, or investigative history.
Data status and what each date means
- Exclusion date — the date in each LEIE record (the EXCLDATE field). It can be years before the file was published.
- LEIE file imported — when this site loaded the file. Statements such as "listed in the file imported on…" refer to this date.
- File timestamp — the Last-Modified time reported by the HHS-OIG download server. It is a server file time, not an official release date.
- HHS-OIG release date — shown only when confirmed from an HHS-OIG source.
- Source last checked — the most recent time this site downloaded the file, whether or not it had changed.
Source & updates
Records are derived from the official downloadable LEIE database published by HHS-OIG and refreshed on a periodic basis. The latest published file is treated as authoritative for current exclusion status. When a record no longer appears in the latest file, its page is labelled "Not in latest imported file", is left out of the site's listings and sitemaps, is marked noindex for search engines, and no longer asserts an exclusion. Absence from a later file does not by itself mean a person or entity was reinstated: a listing can also disappear when HHS-OIG revises its details (for example an address or NPI), in which case the revised listing appears as a separate record.
Correct or remove a record
If you believe a record is inaccurate, misidentifies you, or should be removed, you can submit a correction/removal request. For authoritative status, always consult HHS-OIG directly.
